12.5 mg once daily for 2 days, 25 mg for at least 2 weeks and thereafter 25-50 mg daily

Dosage regimens

Oral, 12.5 milligrams carvedilol, once daily to meals, over the duration of 2 days. Afterwards, oral, 25 milligrams carvedilol, once daily to meals, over the duration of 2 weeks. Afterwards, oral, 25 milligrams carvedilol, 1 to 2 times daily to meals. The maximum allowed total dose is 50 milligrams carvedilol daily.

Detailed description

Once daily dosing is recommended.

Adults

The recommended dose for initiation of therapy is 12.5 mg once a day for the first two days. Thereafter the recommended dosage is 25 mg once a day. Although this is an adequate dose in most patients, if necessary the dose may be titrated up to a recommended daily maximum dose of 50 mg given once a day or in divided doses.

Dose titration should occur at intervals of at least two weeks.

Elderly

An initial dose of 12.5 mg daily is recommended. This has provided satisfactory control in some cases. If the response is inadequate the dose may be titrated up to the recommended daily maximum dose of 50 mg given once a day or in divided doses.

Dosage considerations

It should be taken with food to slow the rate of absorption and reduce the incidence of orthostatic effects.

Carvedilol is a vasodilatory non-selective beta-blocker, which reduces the peripheral vascular resistance by selective alpha1-receptor blockade and suppresses the renin-angiotensin system through non-selective beta-blockade. Plasma renin activity is reduced and fluid retention is rare.
