Phenylephrine interacts in the following cases:
Caution should be applied when administering atomoxetine concurrently with phenylephrine, as there is potential for synergistic pharmacological effects.
Severe hypertension may occur following the use of phenylephrine and atropine or other antimuscarinics.
Drugs which have an effect on α1 adrenoceptors could potentiate (such as clonidine) the vasopressive action of phenylephrine.
Phenylephrine may increase the possibility of arrhythmias in digitalised patients.
Drugs which have an effect on α1 adrenoceptors could inhibit (such as doxazosin) the vasopressive action of phenylephrine.
Due to the increased risk of myocardial fibrosis, phenylephrine should be used with caution during general anesthesia with anesthetic agents, such as halothane, that sensitize the myocardium to sympathomimetics.
The pressor effects of phenylephrine may be slightly reduced by lithium carbonate.
The safety of phenylephrine during pregnancy has not been established. Due to the vasoconstrictive properties of phenylephrine, the product should be used with caution in patients with a history of pre-eclampsia. Administration of phenylephrine in late pregnancy or labour may cause foetal hypoxia and bradycardia.
In view of a possible association of foetal abnormalities with first trimester exposure to phenylephrine, the use of the product during pregnancy should be avoided.
The safety of phenylephrine during lactation has not been established. Excretion of phenylephrine in breast milk appears to be minimal. In view of the lack of data on the use of phenylephrine during lactation, this medicine should not be used during breast feeding.
